يصدر المدير قراراً يحدد فيه نماذج طلب الحصول على البطاقة الشخصية ودفتر العائلة
وتجديدهما والشهادات والمستندات
الواجب ارفاقها والاجراءات التي تتبع للحصول على كل منهما ويعفى طالب أي منها من الصاق
طوابع الواردات على هذه النماذج.
