يترتب على المحضر في جميع الحالات التي يجري فيها التبليغ على احد الوجوه المبينة في
المواد المتقدمة ان يشرح فور وقوع التبليغ على الورقة القضائية الاصلية او نسختها او في ذيل
يلحق بها بيانا بتاريخ التبليغ وكيفية اجرائه وان يذكر فيه اذا امكن اسم وعنوان الشخص الذي كان
معرفاً للشخص المبلغ او البيت الذي علقت عليه الورقة القضائية وان يشهد شاهداً على الاصل.
